We know you have many options when choosing a dental provider and we appreciate that you trust us with your oral care. In preparation for your initial appointment, please complete the electronic forms linked below. If you are unable to access the forms electronically, please arrive at least 15 minutes early to complete them in our office.

Payment Policy

We are a fee-for-service dental practice. Unless another financial option is pre-arranged, payment is due in full on the day of treatment. For your convenience, we accept cash, check, and all major credit cards. We also provide short- and long-term financing. If you are interested in our financing options, this must be determined prior to beginning your treatment.

Insurance Policy

Any services that are covered through your insurance will be reimbursed directly to you rather than to our office. Dental insurance plans often pay less than the actual fee for service, therefore the patient or guarantor is the responsible party for all dental services provided. Dental insurance is a benefit with limitations and should not be expected to cover all costs. 

We are considered an out-of-network practice but will work with all insurance providers to request pre-treatment estimates and submit claims on your behalf.
